Position: Case Manager: Substance Use Services & Treatment Programs
Site: Moosonee
Division: Community Mental Health and Addictions
Reports to: Manager of Community Mental Health
Status: Full time Permanent

Weeneebayko Area Health Authority (WAHA) is looking for an experienced professional to fill the key role of CASE MANAGER: SUBSTANCE USE SERVICES & TREATMENT PROGRAMS.

The Organization
The Weeneebayko Are Health Authority (WAHA) is a unified, integrated First Nations Regional Health Authority and delivers most health services to area residents in the communities of Peawanuck, Attawapiskat, Fort Albany, Kashechewan, Moose Factory, and Moosonee. These services include primary care, nursing service, pre-hospital, acute hospital care, mental health services, diabetes education, dental services, and support services.

Position Profile Requirements:

  • Knowledge of addiction and gambling issues as it relates to therapeutic approaches
  • Keeping current on issues and trends in relation to substance use/problem gambling
  • Knowledge of community/regional resources and Traditional Healing programs in the area
  • Experience working with complex clients, clinical/psychosocial assessments, individual, group, and family counselling in collaboration with child and family services
  • Excellent interpersonal, communication and organizational skills

Key Responsibilities:

  • Provision of case management, including psychiatric consultation services, and counselling services for adults that is consistent with the philosophy, policies, and standards of trauma-informed care established by WAHA
  • Coordinate timely access to mental health, developmental and/or addictions services for adult clients who have complex needs
  • Refer and link clients and caregivers to a range of community resources
  • Ensure clients receive thorough and complete assessments, including completion of all necessary documentation
  • Assist clients in constructing treatment plans that meet client needs, with short- and long-term objectives and follow up services when needed

Qualifications & Experience:

  • Diploma or bachelor’s degree in Social Work, Psychology, or family counselling
  • Two to five years’ experience in community-based adult mental health counselling
  • Registration with Ontario College of Social Work and Social Service Workers (OCSWSSW)
  • Must be willing to travel 
  • Must possess an Ontario driver’s license Class G
  • Ability to communicate in the local native Cree language is an asset
